Job summary

UNC Medical Center seeks an experienced RN to coordinate safe, timely discharges for patients with complex nutrition needs, including enteral feeding and TPN. This full-time, day shift role offers Monday–Friday hours with no weekend requirements, onsite in Chapel Hill, NC.

The Care Manager RN will perform comprehensive assessments, develop patient-centered care plans, manage care transitions, advocate for patients, and collaborate with interdisciplinary teams and payers to ensure high-quality,

Qualifications

Min 2 years RN experience.
GI or surgical nursing experience preferred.
Strong discharge coordination skills.
Excellent communication and teamwork.

Responsibilities

Perform comprehensive care manager assessments using standardized tools to evaluate functional status, cognition, and psychosocial supports.
Collaborate with providers and families to develop individualized, evidence-based care plans.
Coordinate care transitions across settings to ensure continuity and avoid fragmentation.
Communicate with payers and care teams to optimize reimbursement and ensure timely approvals.
Document services accurately in the medical record and support coding as appropriate.
