Job summary

UNC Medical Center in Chapel Hill, NC is seeking an experienced RN to coordinate safe, timely discharges for patients with complex nutrition needs, including enteral feeding and TPN. This is an on-site, day-shift role with no weekends.

The Care Manager RN will perform comprehensive assessments, develop patient-centered care plans, manage transitions across settings, and collaborate with providers, families, and payers. Minimum two years RN experience and NC licensure required.

Qualifications

• Minimum of 2 years of RN experience.
• NC RN licensure required.
• GI or surgical nursing experience preferred.
• Experience coordinating discharge plans and communicating with interdisciplinary teams.

Responsibilities

• Conduct comprehensive care manager assessments using standardized tools.
• Collaborate with providers, families, and the healthcare team on individualized care plans.
• Identify barriers to care and develop interventions.
• Coordinate and facilitate care transitions across settings.
• Coordinate care management tasks with the healthcare team.
• Collaborate with payers and utilization management teams for timely approvals.
• Document and bill for services per regulatory requirements.
• Provide longitudinal care and advocacy within the care team.
