EmblemHealth seeks a care manager to provide care coordination within a multi-disciplinary team. You will perform telephonic or face-to-face assessments, identify gaps in care, and coordinate interventions to support high-risk members in the community.

The role emphasizes collaboration with physicians, nurses, social workers and community resources to improve health outcomes and optimize utilization of services. Flex scheduling may be required.

Formación

• Bachelor's degree required.
• Active and unrestricted RN license in NY state.
• Compact nursing license preferred.
• CCM certification preferred.
• 4–6+ years of clinical experience required.
• Strong communication skills required.
• MS Office proficiency required.
• System knowledge of EMR in a highly automated environment required.

Responsabilidades

• Assess and evaluate needs of our most complex members with a multidisciplinary team.
• Identify goals, strategies and interventions including referrals and education.
• Develop medical management strategies to ensure high-quality care.
• Include member and family as appropriate.
• Engage with the member PCP/designee and treatment team.
• Collaborate with stakeholders to ensure action plan knowledge.
