Job summary

EmblemHealth in New York is seeking a skilled Care Manager to work within a multi-disciplinary team, performing assessments, coordinating care, and supporting high-risk members in the community. You will collaborate with physicians, care coordinators, social workers, and providers to develop and implement individualized care plans, monitor outcomes, and ensure timely, safe, and cost-effective care.

Qualifications

• Active and unrestricted RN license in NY.
• CCM certification preferred.
• 4–6+ years of clinical experience.
• Ability to manage a highly complex caseload.
• Experience in case management, managed care, and/or utilization management.
• Proficient in MS Office and EMR systems.
• Bilingual ability preferred.
• Strong collaboration and problem-solving skills.
• Flexible schedule including evenings, weekends, holidays.

Responsibilities

• Assess needs of our most complex members and identify care gaps.
• Develop and evaluate medical management strategies with the team.
• Engage with members, caregivers, and providers to set attainable goals.
• Coordinate care plans and participate in case conferences.
• Document in EMR within defined timeframes.
