Aufgaben

• Assess and evaluate needs of complex members with multidisciplinary collaboration.
• Identify goals and interventions, including referrals and community resources.
• Develop and evaluate medical management strategies with the care team.
• Engage with members, families, PCPs, and providers to achieve attainable goals.
• Coordinate interventions and ensure compliance with regulations and standards.
• Enter and maintain documentation in the EMR within defined timeframes.
• Provide ongoing monitoring and support for care plan execution.
