Job summary

Humana iCare is seeking an RN Care Manager to join the Family Care Partnership (FCP) program, serving members in Racine, Kenosha, Waukesha, Washington, Ozaukee, Walworth and Sheboygan counties. You will perform nursing assessments, coordinate care, and support interdisciplinary teams to ensure high-quality, cost-effective services.

The role involves field work with travel up to 50%, remote work flexibility, and a compensation range of $71,100 to $97,800 per year with bonus eligibility.

Qualifications

• Registered Nurse licensed in Wisconsin.
• Two years of related health care experience.
• Willingness to travel to multiple counties (Racine, Kenosha, Waukesha, Washington, Ozaukee, Walworth, Sheboygan).
• BSN degree required.
• Previous home health, managed care or case management experience.
• Case Manager Certification preferred.

Responsibilities

• Perform home and community visits to complete nursing assessment and ensure access to services.
• Conduct nursing portion of comprehensive assessment and provide input to care plans.
• Monitor wellness and health promotion activities based on assessments.
• Re-evaluate health at intervals to manage ongoing issues and responses to interventions.
• Assess safety and medication management, and provide or arrange needed services.
