Job summary

Behavioral Health Network (BHN) seeks a Nurse Care Manager to join the CBHC Care Management team, advancing mission-driven behavioral health care.

Qualifications

• RN license is required.
• 1-2 years' work experience in behavioral health, human services, or medical field.
• Demonstrates cultural and linguistic competence with diverse populations.
• Ability to provide nonjudgmental support.
• Knowledge of motivational interviewing techniques.
• Familiarity with CBHC policies and procedures and use of EHRs.

What You Will Do

• Serve as a strong advocate for individuals served, ensuring their voices are heard and their needs are met, while promoting their autonomy and self-determination.
• Secure appropriate services for individuals served and monitor the progress of services to ensure quality care.
• Identify safety issues and take appropriate measures to ensure individuals served's safety and well-being in consultation with supervisor, program leadership, and/or Emergency Services.
• Build and maintain strong relationships with community partners, including social service agencies, healthcare and behavioral health providers, and other relevant organizations.
• Complete and maintain accurate and timely documentation of interactions, progress, and referrals, and complete necessary reports for funding agencies and stakeholders.
• Work to reduce utilization of higher levels of care when appropriate, and support the transition of individuals served from inpatient settings back into the community.
• Review initial and annual MDS assessments in collaboration with the care team to identify health needs, safety concerns, and gaps in care.
• Implement care plans that reflect individual preference and lead to improved quality of life, in collaboration with individuals served and assigned Care Coordinators.
• Provide care management services, including post-hospitalization follow-up and medication reconciliation.
• Provide outreach to individuals served in the home, office, or community to assess and address health needs and facilitate improved health outcomes.
• Perform a minimum of one monthly face-to-face outreach visit per assigned caseload.
• Evaluate individuals served' medical history and diagnoses for accuracy, and assess health needs, history, and existing support networks.
• Assist the care team in creating person-centered care plans and scheduling appointments as needed.
• Educate individuals served and their families/supports on relevant health-related matters.
• Monitor outcomes and collaborate with medical and/or behavioral health providers to ensure continuity of care.
• Become familiar with CBHC policies & procedures and adhere to any updates or changes.
• Consult with community agencies, providers, and informal supports to maintain coordination in the treatment process.
• Submit referrals and/or authorizations to request services indicated by medical and/or behavioral health needs and care plans.
• Provide formal and informal training to the care team regarding health/medical issues facing individuals served.

Who You Are

• High School Diploma or GED required.
• Registered Nurse license required.
• 1-2 years' work experience in a related behavioral health, human services, and/or medical field required.
• Demonstrates cultural and linguistic competence in working with individuals of diverse cultural backgrounds and linguistic needs.
• Demonstrates genuine care and concern for individuals served, acknowledging their struggles and validating their experiences.
• Ability to provide support without judgment, recognizing that each individual's situation is unique and complex.
• Knowledge of and ability to utilize motivational interviewing techniques to help individuals served identify strengths and set goals.
• Working knowledge of community resources and how to help individuals served access them.
• Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management.
• Familiarity with Internet, email systems, and electronic health records (EHRs).
• Valid driver's license and reliable, legally registered vehicle available for all working hours required.

Salary Range: $75,000-$85,000
