Job summary

Mercy Health is seeking a dedicated RN Care Manager in Springfield to coordinate patient care across the continuum and ensure seamless transitions. You will assess needs, develop care plans, and collaborate with the healthcare team to optimize outcomes and resource use.

The role emphasizes discharge planning, documentation, and active participation in interdisciplinary rounds to improve patient experiences and reduce readmissions.

Qualifications

• Must hold BSN (preferred for BSMH, required for RSFH)
• RN licensure in state of practice or compact
• ACM/CCM/ANCC Nursing Case Management certification preferred

Responsibilities

• Coordinate care across continuum to ensure safe, timely transitions
• Identify and assess patients needing active case management
• Document care plans and handoffs clearly and promptly
• Support Advance Care Planning, length of stay management and readmission prevention
• Collaborate with care team to address barriers to progression of care
• Promote proactive, resourceful care and discharge planning
