Job summary

AdventHealth in Denver, CO seeks an experienced RN Case Manager to coordinate discharge planning and ensure timely transitions from hospital to post-acute care, addressing medical, social, and financial needs of patients and families.

You will participate in multidisciplinary rounds, educate patients and families, assess readmission drivers, and develop comprehensive discharge plans with contingency arrangements and post-acute services. RN license and 2+ years nursing experience required.

Qualifications

• RN licensure and an Associates degree are required.
• Bachelor's degree in Nursing preferred.
• Minimum 2+ years of medical/hospital nursing experience.
• Experience with discharge planning and case management strongly preferred.

Responsibilities

• Lead discharge planning through multidisciplinary rounds and coordinate post-acute resources.
• Educate patients and families about medical, social, and financial implications of illness.
• Assess readmission factors and develop interventions to prevent avoidable days.
• Document discharge plans, barriers, and authorizations for post-acute care.
• Communicate with payors for post-acute care authorizations as needed.
• Review medical records to integrate clinical, social, and financial factors.
