Job summary

AdventHealth in La Grange, IL is seeking a Registered Nurse Case Manager (PRN) to join the care management team. The role requires coordinating multidisciplinary rounds, discharge planning, and patient-family education across inpatient and post-acute settings.

You will assess readmission risks, collaborate on care conferences, and ensure timely transitions while engaging with Payors for post-acute needs and maintaining comprehensive documentation.

Qualifications

• 2+ medical/hospital nursing experience.
• Care management/utilization management experience preferred.
• RN with BLS and CCM/ACM certifications preferred.

Responsibilities

• Participates in multidisciplinary rounds to review changes in patient status and discharge plans.
• Educates patients and families on illness impact and connects resources.
• Assesses readmissions and reasons with the care team.
• Leads patient/family care conferences with the team.
• Documents discharge planning evaluations and barriers to progression of care.
• Coordinates post-acute care arrangements and communicates needs to Payors.
• Reviews medical records to integrate clinical and social factors in discharge plans.
• Develops contingency discharge plans and ensures timely care coordination.
• Leverages technology to ensure continuity of care information is complete.
