Job summary

Recruitpro is seeking an experienced Nurse Care Manager to support clients in a part-time capacity in New Jersey. The role focuses on coordinating care, performing health screenings, and working with prescribers to optimize treatment plans.

The successful candidate will hold RN licensure, have experience in nursing or nursing case management, and demonstrate strong communication, organization, and collaboration with insurers and providers to ensure access to care.

Qualifications

• Knowledge of health care issues pertaining to substance use disorders and related conditions (HIV/AIDS).
• Accurate regulatory record keeping is required.
• Knowledge of nutritional requirements and practices.
• Therapeutic communication techniques are required.
• Ability to work with clients to gain trust while maintaining boundaries.
• Excellent prioritization and time management skills.
• Excellent organizational and detail-oriented skills.
• Ability to establish and maintain effective working relationships with clients, staff, providers and the public.
• Microsoft Office (Word, Outlook) proficiency.

Responsibilities

• Coordinate active care coordination for clients, including scheduling and follow-up.
• Develop protocols for health screening and assessment.
• Screen hepatitis C, HIV, and related conditions at admission and intervals.
• Refer clients to providers for vaccinations and medications as appropriate.
• Interface with Medicaid and private insurers to secure coverage information.
• Ensure high-quality, evidence-based care and preventative services.
• Collaborate with specialty services for referrals and record sharing.
• Follow up on care plans and address barriers to care.
