Job summary

GRNH, LLC is seeking a Care Coordinator for our New Brunswick, NJ location. The role focuses on ensuring a safe, supportive environment for clients in the program, coordinating with 3rd parties for aftercare, and assisting with discharge planning.

Qualifications include a graduate degree/education level with preference for Bachelor's, and experience in substance use or mental health fields. CPR certification is preferred and privacy laws must be observed.

Qualifications

• Bachelor's degree preferred; minimum requirement of High School Diploma or equivalent
• Experience in substance use or mental health setting recommended but not required
• CPR certification preferred

Responsibilities

• Coordinate with 3rd parties to connect clients to appropriate aftercare settings
• Direct case managers to provide documentation for external parties (e.g., courts, FMLA)
• Communicate with clinical/medical teams to direct aftercare plans
• Inform family within 24 hours to discuss treatment recommendations and arrange auxiliary support
• Assist with discharge planning and documentation, including notifying staff of discharge dates
• Ensure privacy of client information per GRNH, LLC policies and federal/state laws
