Recruitpro is seeking a Nurse Care Manager in Vineland, NJ for a part-time, hourly role within the Nursing department. The position focuses on coordinating care, screening for hepatitis C/HIV, and interfacing with prescribers and insurers to secure services and coverage.

The successful candidate will manage care plans, monitor medications for safety, and maintain thorough documentation while collaborating with a multidisciplinary team.

Competenze

• RN licensure and at least two years of nursing or nursing case management experience.
• Experience in addiction-related health care is preferred.
• Strong record-keeping and regulatory compliance abilities.

Mansioni

• Coordinate active care plans and assist with appointments and referrals.
• Screen and coordinate health services for clients with hepatitis C, HIV, and related conditions.
• Collaborate with prescribers and insurers to secure coverage.
• Monitor medications, prevent polypharmacy, and adjust care plans as needed.
