Job summary

Orlando Health Lake Mary Hospital seeks a Registered Nurse Care Manager, Acute to promote and coordinate care from admission to discharge. You will collaborate with the clinical team to identify patients who benefit from care coordination, assess risk factors, and ensure appropriate utilization and transitions.

Qualified candidates hold a Florida RN license and have experience in care management, utilization management, and complex case coordination.

Qualifications

• Graduate of an approved school of nursing.
• Three years of experience in chronic disease management, care management, care coordination, utilization management, or acute clinical care.
• Must hold and maintain a current Florida RN license.
• Handle with Care (HWC) Certification is required for the Behavioral Health Unit within 90 days of hire.

Responsibilities

• Assess patients and determine medical necessity for review.
• Coordinate care across disciplines and settings, including transitions of care.
• Lead and participate in multidisciplinary patient care conferences.
• Monitor length of stay and plan transitions with InterQual guidelines.
• Communicate plan of care with patients, families, and the care team.
