Job summary

University of Maryland Medical System is seeking a Care Coordinator (RN) to support optimal health outcomes and timely access to care. The role emphasizes coordination across clinical teams, payer requirements, and post-acute services, with varied shifts to meet patient needs.

The ideal candidate holds an MD RN license with at least three years in acute care, strong communication, and collaboration skills to improve admission, stay reviews, and transitions of care within a comprehensive hospital

Qualifications

• RN license in Maryland with current credentials.
• Three years of experience in an acute care hospital.
• Experience in case management and/or utilization management preferred.
• Excellent verbal and written communication, interpersonal and organizational skills.

Responsibilities

• Assess patients for clinical, psychosocial, financial and operational factors affecting care.
• Collaborate with clinical team to reduce inpatient length of stay and readmissions.
• Perform and document admission and continued stay reviews using InterQual/Milliman criteria.
• Develop effective transition plans based on patient needs and available resources.
• Participate in CTR rounds and integrate patient narrative into the care plan.
• Develop corrective action plans for complex issues and escalate as needed.
• Document information per care coordination policy, ensuring complementarity to the care plan.
• Collaborate with Physician Advisor on complex medical plan issues.
• Participate in clinical performance improvement activities and stay current on practice/protocols.
