Ohio State University Physicians is seeking a Clinical Nurse Care Coordinator to collaborate with clinic teams and guide patients through care plans. The role emphasizes high-risk and complex patient populations, including transitions from pediatric to adult care and post-discharge follow-up.

The RN will liaise with families and other agencies, ensure adherence to standards, and participate in program initiatives to enhance comprehensive, whole-person care.

Qualifications

• Associate Degree in Nursing from a CCNE or ACEN accredited nursing program.
• Current Ohio Registered Nurse license.
• CPR/BLS certification.
• Knowledge of triage procedures and clinic/urgent care experience.
• Ability to maintain quality, safety, and infection control standards.
• Ability to educate and clearly communicate with patients and families.

Responsibilities

• Collaborate with clinic team to provide evidence-based, patient-centered care.
• Focus on high-risk medically complex patients and those in care transitions.
