Job summary

Orlando Health Medical Group is seeking an RN Care Coordinator for the General Surgery Trauma and Burn Clinic in Orlando, FL. This full-time, 40-hour position reports to the department and works Monday–Friday, 8am–5pm at 77 W. Underwood St, Orlando, FL 32806.

The roleCollaborates with the assigned clinical team to identify patients who will benefit from care coordination, assessing risk factors and coordinating clinical utilization management and preventive services.

Qualifications

• Effective July 1, 2024: New Hires and team members moved into this job and/ or transferring departments must have obtained a Bachelor of Science in Nursing (BSN) degree.
• Maintains current license as an RN in the State of Florida.
• Maintains current BLS/ healthcare provider.

Responsibilities

• Promotes wellness, restores health, and supports end-of-life care in ambulatory care nursing practice.
• Assesses patients’ needs and develops an individualized plan of care.
• Provides care appropriate to the age of patients served on the unit.
• Delivers and documents patient care, education, medications, treatments, and safety.
• Demonstrates competence with unit equipment and nursing skills.
• Prioritizes patient care in line with evidence-based standards of care.
• Communicates patient condition and unit concerns to the care team.
• Maintains a safe and caring environment for patients and families.
