Job summary

Orlando Health, Inc. is seeking a Nurse Care Coordinator to promote and facilitate effective management of hospital resources from admission to discharge.

The role collaborates with the clinical team to identify patients who will benefit from care coordination, assess risk factors, and support transitions to appropriate levels of care. Responsibilities include assessing patients, coordinating care, leading multidisciplinary conferences, and ensuring compliance with regulatory standards.

Qualifications

• Graduate of an approved school of nursing.
• Three (3) years of experience in chronic disease management, care management, care coordination, utilization management, or acute clinical care.
• Maintains current Florida RN license and BLS/Healthcare Provider certification are required.
• BLS/Healthcare Provider Certification within 90 days of hire.

Responsibilities

• Assess patients within assigned population and evaluate medical necessity.
• Coordinate care and document treatment plans.
• Lead multidisciplinary patient care conferences.
• Ensure compliance with regulatory standards and utilization management.
• Communicate care plan with patients and families.
• Identify and refer complex patients to Social Work Services.
