Job summary

Orlando Health, Inc. is seeking a Nursing Care Manager to promote effective management of hospital resources from admission to discharge.

You will collaborate with the clinical team to identify patients who benefit most from care coordination, assess risk factors, and facilitate transitions to appropriate levels of care. Qualifications

• Graduate of an approved school of nursing.
• Must hold and maintain a Florida RN license; Handle with Care training provided onsite within 90 days for Behavioral Health Unit.
• Minimum of three years in chronic disease management, care coordination, or related acute care settings.

Responsibilities

• Accurate medical necessity screening and submission for Physician Advisor review.
• Care coordination from admission to discharge with the clinical team.
• Leading multi-disciplinary patient care conferences and transition planning.
• Identify and refer complex patients to Social Work Services and other departments.
• Ensures compliance with regulatory standards including Federal, State, Local and Joint Commission.
