Family Pillars Homecare, LLC in Florida is seeking a clinically skilled Registered Nurse to manage and coordinate home care services across Southwest Florida. • Active Florida RN license required.
• Strong clinical assessment, care planning, and patient monitoring skills.
• Ability to coordinate care across managed care, workers’ compensation, and private-pay cases.
• Understanding of service authorizations and ability to track expirations.
• Strong professional communication with patients, families, providers, payers, and adjusters.
• Excellent organization, documentation, time management, and follow-through.
• Proficient with electronic documentation systems and remote communication tools.
• Ability to work independently and manage a geographically dispersed caseload.
• Willingness to travel throughout Southwest Florida for patient visits.

Responsabilidades

• Manage a caseload of managed care, workers’ compensation, and private-pay patients throughout Southwest Florida.
• Review and update individualized care plans in collaboration with patients, families, caregivers, providers, and other care team members.
• Conduct in-home assessments and patient visits as needed based on clinical needs and case requirements.
• Monitor changes in patient condition, identify concerns, and coordinate timely follow-up.
• Coordinate services within approved authorizations, including monitoring hours, visits, service dates, and requests for additional care.
• Communicate with managed care representatives, workers’ compensation adjusters, external case managers, and treating providers regarding patient progress.
• Coordinate private-pay services with patients and families, communicating the care plan and changes in needs.
• Maintain accurate, timely clinical documentation and case updates meeting agency and payer requirements.
• Collaborate with scheduling, HR, and clinical leadership to address staffing needs and support continuity of care.
• Assist with onboarding and orientation of field staff assigned to cases.
• Identify and address barriers to care, service gaps, and patient/family concerns.
• Assist in daily Quality Assurance of notes and files, including SOCs, evaluations, and all clinical visits including Therapy.
