Job summary

Family Pillars Homecare, LLC is seeking an organized, clinically skilled Registered Nurse to manage and coordinate home care services for patients across Southwest Florida. The role is primarily remote with in-person patient visits as needed, serving as the clinical point of contact for patients, families, caregivers, therapy providers, payers, and adjusters.

Qualifications

• Active Florida RN license.
• Strong clinical assessment and care planning.
• Experience coordinating managed care, workers’ compensation, and private-pay cases.
• Ability to travel throughout Southwest Florida.

Responsibilities

• Manage a caseload of managed care, WC, and private-pay patients in Southwest Florida.
• Review and update individualized care plans with patients and care team.
• Conduct in-home assessments and patient visits as needed.
• Monitor patient condition and coordinate timely follow-up.
• Coordinate services within authorizations, monitor hours and dates.
• Communicate with payers, adjusters, and providers on progress.
• Ensure accurate clinical documentation and file updates.
• Support staffing and onboarding with clinical leadership.
