Paoli Hospital, part of Main Line Health, seeks an RN Care Coordinator to orchestrate care across the continuum. You will partner with physicians, families, and colleagues to develop, implement, and monitor comprehensive care plans focusing on health promotion and disease management.

The role emphasizes committee involvement and evidence-based projects, with Per Diem, weekends and some weekdays to cover. Active PA RN license is required.

Competenze

• Three years of recent clinical experience required.
• Leadership experience preferred.
• Associates Degree OR BSN required upon hire; if Associates, BSN required within 3 years of hire
• Master's Degree preferred.

Position:Registered Nurse (RN) - Care Coordination

Shift:Per Diem, Varied Shifts; mainly weekends, some weekdays to cover. 16 hours/month plus one weekend per month and one holiday per year.

Experience:

1. Three years of recent clinical experience required.
2. Leadership experience preferred.

Education:

1. Associates Degree OR BSN required upon hire; if Associates, BSN required within 3 years of hire
2. Master's Degree preferred.

Licensures/Certifications:

Active PA RN license required
