Job summary

RN Care Coordinator position at Paoli Hospital within Main Line Health is focused on guiding patients through the care continuum with physicians, peers, and community colleagues. • Act as advocate and liaison across the care continuum to ensure high-quality, cost-effective care.

Position:

Registered Nurse (RN) - Care Coordination

Shift:

Per Diem, Varied Shifts; mainly weekends, some weekdays to cover. 16 hours/month plus one weekend per month and one holiday per year.

Experience:

• Three years of recent clinical experience required.
• Leadership experience preferred.

Education:

• Associates Degree OR BSN required upon hire; if Associates, BSN required within 3 years of hire
• Master's Degree preferred.

Licensures/Certifications:

Active PA RN license required
