Job summary

McLaren Health Care in Grand Blanc, MI seeks an RN Care Manager to coordinate comprehensive, patient-centered care for chronically ill populations within an interdisciplinary team.

You will assess needs, develop individualized care plans, provide self-management support, and collaborate with PCPs, specialists, and community partners to improve outcomes and reduce hospitalizations. Requires Michigan RN license and 3 years' chronic care experience.

Qualifications

• RN with a valid unrestricted Michigan license.
• Three (3) years clinical nursing experience serving chronically ill patients.
• Extensive knowledge of issues associated with chronic care and geriatrics.
• Preferred: BSN; Three (3) years in health plan/Physician Organization care coordination/management.

Responsibilities

• Provides telephonic and face-to-face comprehensive assessment and care management services to patients as part of an interdisciplinary team.
• Uses multi-dimensional assessment skills and risk tools to target high-risk populations.
• Develops comprehensive individualized plans of care and targeted interventions.
• Coordinates patient care with PCPs, specialists, McLaren Health Care, and community resources.
• Monitors patient/family response to plan of care and revises the care plan as indicated.
• Maintains required documentation of all care management activities and supports patient self-management.
