Job summary

Housing Works is seeking a Staff Nurse (RN) in New York to deliver comprehensive nursing care in accordance with the physician’s plan of care and Housing Works policies.

The role involves assessing, planning, providing and documenting nursing care for a designated patient group, coordinating with interdisciplinary teams, and maintaining detailed clinical documentation. A NY RN license and BSN are required; HIV/AIDS experience plus; health plans and PTO are provided.

Qualifications

• Must be a Registered Nurse, licensed by the State of New York.
• Bachelor’s degree from an accredited college or university.
• Experience with persons with HIV/AIDS, histories of substance use and/or mental illness a plus.
• Experience working in a setting that promotes interdisciplinary care planning.
• Bilingual preferred (Spanish/ English ).

Responsibilities

• Coordinates all necessary client services with other interdisciplinary team members in the comprehensive care planning process.
• Performs specialized care and demonstrates initiative in learning new skills and developing competency.
• Maintains accurate documentation of nursing activities including medication profile and reports changes to the physician.
• Completes clear, concise clinical notes documenting skilled services and other documentation with standard abbreviations.
• Communicates necessary information to the Nurse Care Coordinator.
• Attends patient case conferences to coordinate and ensure continuity of care.
• Works with patient and health care team to coordinate services and promote quality care and positive outcomes.
• Provides on-call coverage when required.
• Establishes and maintains relationships with physicians" offices, hospitals, clinics to ensure flow of information.
• Coordinates patient medical care with external primary health care providers.
