Job summary

Housing Works seeks a Patient Care Coordinator to screen patients, verify insurance, and provide medical case management for Primary Care patients in New York. You will coordinate intake, PRAPARE assessments, and referrals, while ensuring documentation is accurate in EMR systems.

You will work with a multidisciplinary team, participate in quality improvement meetings, and help advocate for housing and health services for people living with HIV/AIDS.

Qualifications

• Strong understanding of HIV/AIDS, case management, and entitlements.
• Proficiency with HIPAA regulations governing medical documents.
• Spanish language skills are preferred for client interactions.
• Bachelor’s degree preferred for this role.

Responsibilities

• Coordinate intake screenings to determine patient eligibility for services.
• Conduct PRAPARE assessments and provide referrals for primary care clients.
• Document hospitalizations and schedule discharge appointments as needed.
• Collaborate with care management and program partners for referrals.
• Maintain client progress documentation and support regular meetings.
