Job summary

Covenant Health is seeking a Registered Nurse Care Coordinator II in Knoxville, TN, to integrate evidence-based practice into patient care, coordinate staff education, and serve as a clinical resource for the health care team. The role reports to the Manager of Quality Management at the corporate level.

The position emphasizes care coordination, guideline development, and continuous quality improvement for the designated patient population within Covenant Medical Group.

Qualifications

• Bachelor’s degree in Nursing or related field or certification as listed below.
• Four (4) years of acute care nursing experience; minimum of three (3) years in area of assigned responsibility.
• Current Tennessee RN License.
• Current certification in Case Management, CCM, ACM or CPHQ or Bachelor’s degree in Nursing or related field.

Responsibilities

• Utilizes case finding criteria to screen patients and develop a comprehensive plan.
• Facilitates timely documentation review with the CDI specialist to guide the care team.
• Utilizes the care coordination process to evaluate progress with patient, caregivers and provider.
• Modifies the case management plan to meet changing patient needs and secures resources.
• Researches, designs and implements practice guidelines in collaboration with providers and nursing staff.
• Documents results of communication in the patient’s medical record and communicates goals and transition plans.
