Allina Health is seeking an RN for Home Care in Minnesota. The role emphasizes comprehensive patient assessments, care planning, and collaboration with families to achieve measurable outcomes.

The nurse will maintain clinical competencies and apply evidence-based practice within regulatory guidelines. Responsibilities include coordinating with the care team, potentially serving as Charge Nurse or Preceptor, and fulfilling organizational requirements.

Competenze

• Associate's or Vocational degree required.
• 2+ years RN experience required.
• Bachelor's degree preferred.
• Case Management experience preferred.

• Comprehensive assessment of patient and family needs; establishment of outcomes and plan of care; supervision and modification as needed.
• Maintains education and competencies to provide high quality patient care.
• Incorporates evidence-based nursing practice considering current statutes, rules and regulations when developing the plan of care.
• Establishes realistic and measurable patient outcomes in collaboration with patient and family.
• Coordinates, collaborates, and delegates to ensure safe, effective care.
• Fulfills all organizational requirements.
