Job summary

Allina Health Hospice is seeking a Case Manager RN for the Coon Rapids area. This role focuses on comprehensive patient assessments, care planning, and collaboration with the care team to coordinate in-home nursing services.

You will work with patients and families to establish achievable outcomes, ensure safety, and provide guidance across the continuum of care. Travel is required with mileage reimbursement; 1.0 FTE with 40 hours per week and every 4th weekend rotation.

Qualifications

• Active RN licensure in MN or WI upon hire.
• Current BLS certification (AHA or Red Cross) or in-house training.
• CHPN or CMGT-BC preferred and beneficial.

Responsibilities

• Perform comprehensive patient and family assessments and planning.
• Develop and adjust the plan of care with measurable goals.
• Coordinate care, delegate tasks, and collaborate with the team.
• Maintain competencies and provide high quality in-home nursing care.
