Job summary

Qualifications

• Requires Baccalaureate Degree in Nursing or Bachelor's in biological/health/social sciences.
• Current RN licensure in Virginia or eligibility.
• Minimum of three years of nursing experience in an acute care setting.

Responsibilities

• Works with attending and consulting physicians to facilitate transitions through hospitalization.
• Collaborates with multi-disciplinary team to coordinate care across continuum and ensure appropriate reimbursement.
• Seeks social work expertise to resolve psychosocial issues and develop complex transition/discharge plans.
• Acts as liaison between hospital and community agencies for information exchange.
• Maintains compliance with QIO and CMS guidelines; uses medical necessity criteria to assess care level and setting.
• Participates in patient safety initiatives and upholds VCUHS Mission and Values.
• Establishes and maintains professional rapport with providers, patients/families, and internal customers.
