Job summary

VCU Health in Richmond, VA seeks a seasoned RN Care Coordinator to drive safe, efficient transitions from hospital to next care setting. You will collaborate with physicians, social workers, and payers to ensure high-quality, cost-effective care through the acute episode and post-discharge period.

The role requires Virginia RN licensure (or eligibility), a BSN, 3+ years in acute care, and CEUs; a MSN or related master’s degree is preferred.

Qualifications

• Licensed registered nurse in the State of Virginia or eligible.
• Bachelor's degree in Nursing from an accredited School of Nursing.
• Complete 15 Continuing Education Units per year.
• Minimum of three (3) years of nursing experience in an acute care setting.

Responsibilities

• Works with attending and consulting physicians to facilitate effective and efficient transition through hospitalization.
• Collaborates with all members of the multi-disciplinary team to ensure patient needs are met and care delivery is coordinated across the continuum.
• Seeks the expertise of social workers to resolve psychosocial patient care issues and to develop complex patient transition/discharge plans as needed.
• Interacts with patients, family members, healthcare professionals, community and state agencies in this effort.
• Serves as a liaison between the hospital and community agencies or facilities for exchange of clinical and referral information.
• Maintains hospital compliance with Quality Improvement Organization (QIO) series and CMS guidelines.

Baccalaureate Degree in Nursing
