Phpmcs in Colorado Springs seeks a Registered Nurse Care Coordinator to deliver comprehensive care coordination within a Primary Care setting for at-risk members. You will assess needs, plan care, monitor progress, and coordinate with care teams and community resources to improve outcomes.

You will provide crisis intervention, empower self-management, and document care using PHP Care Coordination procedures while ensuring HIPAA compliance and strong collaboration across departments.

Formación

• RN with valid license in good standing.
• Experience with high-risk DSNP/CSNP/Medicaid preferred.
• 1+ year of case management or related healthcare experience.
• Ability to collaborate and communicate with care teams.
• Maintain HIPAA compliance and professional licensure.

Responsabilidades

• Perform comprehensive assessments to identify needs and barriers.
• Develop and implement member-centered care plans with teams.
• Coordinate with PC providers and care teams for continuity.
• Participate in care rounds and document updates for care plans.
• Provide crisis intervention and motivational interviewing to engage members.
