Job summary

OSUP Community Medical Group, part of the Ohio State University Wexner Medical Center, seeks a Clinical Nurse Care Coordinator to support patient-centered care across clinics. You will help high-risk patients stay current with care plans and goals, coordinating with care teams to ensure seamless transitions and comprehensive support.

The role emphasizes collaboration with families, strong communication, and adherence to evolving standards to deliver high-quality, whole-person care.

Qualifications

• Associate Degree in Nursing from a CCNE/ACEN accredited program.
• Current Ohio RN license.
• CPR/BLS certification.
• Knowledge of triage procedures in clinic, physician office or urgent care.
• Ability to educate patients and families about disease and treatment.
• Strong written and verbal communication skills.

Responsibilities

• Collaborate with clinic teams to provide evidence-based, patient-centered care.
• Special focus on high-risk medically complex patients and care transitions.
• Lie with families and other agencies to improve overall communication and coordination.
• Stay up to date on clinical standards and support programs and processes.
• Assist with quality improvement efforts and infection control compliance.
