Job summary

OSUP Community Medical Group seeks a Clinical Nurse Care Coordinator to support high‑risk patients and care transitions. You will collaborate with clinic teams to ensure evidence‑based, patient‑centered care and keep care plans aligned with patient goals.

Responsibilities include liaising with families and other agencies, applying current nursing standards, and assisting programs to improve health outcomes. Ohio RN license and CPR/BLS are required.

Qualifications

• Associate Degree in Nursing from a CCNE or Accreditation Commission for Education in Nursing accredited nursing program.
• Current Ohio Registered Nurse license.
• CPR/BLS certified.
• Knowledge of triage procedures along with clinic, physician office and/or urgent care experience.
• Ability to maintain quality, safety, and/or infection control standards.
• Knowledge of appropriate procedures and standards for the administration of medications and patient care aids.
• Ability to educate and clearly communicate with patients and/or families as to the nature of disease and to provide instruction on proper care and treatment.
• Ability to convey information both in writing and verbally.
• High level of interpersonal skills to handle sensitive and confidential situations.
• Position continually requires demonstrated poise, tact and diplomacy.
• Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions.

Responsibilities

• Keeps patients on track with their care plans and related goals.
• Focuses on high-risk medically complex patients, disease registries, and care transitions.
• Acts as a liaison with family and other agencies to improve communication.
• Stays up to date on standards and assists with programs and processes.
