Job summary

Ohio State University Physicians is seeking a Clinical Nurse Care Coordinator to lead a dynamic team in delivering evidence-based, patient-centered care. You will focus on high-risk, medically complex patients and those transitioning between care settings, coordinating with families and agencies to ensure comprehensive, whole-person care.

The role requires an Associate Degree in Nursing with an Ohio RN license and CPR/BLS certification.

Qualifications

• Associate Degree in Nursing from a CCNE or ACEN accredited program.
• Current Ohio Registered Nurse license.
• CPR/BLS certified.
• Knowledge of triage procedures along with clinic, physician office and/or urgent care experience.
• Ability to maintain quality, safety, and/or infection control standards.
• Knowledge of medications administration and patient care aids.
• Ability to educate and clearly communicate with patients and/or families as to the nature of disease and to provide instruction on proper care and treatment.
• Ability to convey information both in writing and verbally.
• High level of interpersonal skills to handle sensitive and confidential situations.
• Position continually requires demonstrated poise, tact and diplomacy.
• Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions.

Responsibilities

• Collaborate with clinic team to provide evidence-based patient-centered care and keep care plans up to date.
• Focus on high-risk medically complex patients and those in care transitions.
• Act as a liaison with family members and other agencies to improve communication for comprehensive care.
• Stay up to date on standards and assist with programs and processes.
