Location: Boston Area (Field-Based with Home Office)



  • Worcester and surrounding areas, MA

  • Lowell, Merrimack Valley, MA

  • Braintree and surrounding areas, MA

  • Boston Market, MA

  • Allston MA Region, Boston, MA (Bilingual Cantonese required)

  • Allston MA Market, Boston, MA

  • Boston, MA and surrounding areas

  • West Roxbury, Boston, MA (Bilingual Cantonese required)

  • Lawrence MA surrounding area


Schedule: Monday through Friday (No Weekends/Holidays)


Role Overview:



  • Coordinate long-term care needs for patients in the local community.

  • Relay member needs and advocate for optimal care.

  • Work remotely if within a commutable distance of the Boston Area.


Primary Responsibilities:



  • Assess, plan, and implement individualized care strategies.

  • Initiate referrals for social service programs.

  • Manage care plans as a single point of contact.

  • Communicate health-related information to ensure coordinated care.

  • Advocate for patients and families.


Qualifications:



  • Current RN licensure in Massachusetts.

  • 2+ years of clinical experience.

  • 1+ years of experience with MS Office.

  • Ability to travel within the assigned region.


Preferred Qualifications:



  • BSN, Master's Degree, or higher in Clinical Field.

  • CCM Certification.

  • 1+ years of community case management experience.

  • Experience in team-based care and managed care.
