Job summary

OptumCare is seeking a Registered Nurse to join the primary care team in Washington state. The role involves telephonic and in-person member support across the Lewis County area, with a focus on care coordination and adherence to CPM models of care.

The position emphasizes a Monday–Friday schedule with no nights, weekends, or on-call shifts, and includes a $7,500 sign-on bonus for external candidates.

Qualifications

• Current unrestricted RN license in WA state.
• 3+ years of RN experience.
• 1+ years of case management experience.
• Experience with using electronic medical records.
• Basic Microsoft Office skills; able to navigate Windows.
• Proven excellent communication and customer service skills.
• Valid driver's license and ability to travel locally 75%

Responsibilities

• Establish and maintain communication with ICT, member/RP and primary caregiver staff in the ALC and APC.
• Coordinate and liaise with utilization review, provider relations, pharmacy and other operational units for seamless benefits coordination.
• Review member status and adjust ICP as necessary to meet goals.
• Perform administrative functions for the PCP and/or APC.
• Participate in program effectiveness analysis and planning.
• Perform member visits and rounding as established by the APC.
• Communicate member status with the APC for evaluation and ICP updates.
• Coordinate with the member, family/RP and primary caregivers during each visit.
