Job summary

OptumCare in Washington State is seeking a Registered Nurse to join the primary care team and support APC/ISNP/IESNP care coordination. The role requires 75% patient visits in the Pierce County area, with no night or weekend shifts and a $7,500 sign-on bonus for external candidates.

You will work under the provider and Clinical Services Manager, performing telephonic and in-person visits and contributing to care plans, data collection, and ICP updates across the continuum.

Qualifications

• Current Washington RN license (unrestricted).
• 3+ years of RN experience.
• 1+ years of case management experience.
• Experience using electronic medical records (EMR).
• Proficient in Microsoft Office and Windows navigation.

Responsibilities

• Support CPT/ICP goals within the APC and ISNP/IESNP models.
• Provide telephonic and in-person care coordination with the team.
• Coordinate and communicate with multiple care and administrative units.
• Maintain patient confidentiality and HIPAA compliance.
