Job summary

UnityPoint Health is seeking an RN Care Coordinator for the PACE Clinic, Monday–Friday 8:00 AM–4:30 PM with weekend rotation. The role focuses on coordinating, implementing, and evaluating participant-centered care within the PACE interdisciplinary team to ensure high-quality, cost-effective services across the continuum.

Qualifications

• Degree/diploma in nursing.
• Two years clinical nursing experience preferred.
• Current Iowa RN license and CPR certification.
• Valid driver’s license and auto insurance.

Responsibilities

• Conduct nursing assessments and develop individualized care plans.
• Coordinate care across clinic, home care, and hospital settings with the IDT.
• Provide skilled nursing care including visits, triage, wound care, and medications.
• Serve as primary nurse contact for assigned participants and collaborate with the care team.
