Job summary

Senior Care Partners PACE seeks a compassionate Charge Registered Nurse to lead daily clinical operations and coordinate care for participants onsite at a PACE Center.

This RN role provides clinical leadership, oversees day-to-day nursing operations, and collaborates with an interdisciplinary care team to ensure high-quality, participant-centered care for frail elders in the community.

Qualifications

• Current RN license and graduation from an accredited nursing program.
• Two years of Charge Nurse or leadership experience preferred.
• At least one year of experience with frail elderly or long-term care.
• Current CPR certification (AHA) or ability to obtain.
• Strong clinical judgment, communication, organizational, and leadership skills.
• Commitment to compassionate, participant-focused care.

Responsibilities

• Coordinate and oversee daily clinical care activities and workflows.
• Provide leadership and support to the clinical care team.
• Monitor and manage participants’ acute and chronic health conditions with Primary Care Providers.
• Administer medications, treatments, immunizations, and wound care as prescribed.
• Conduct nursing assessments and contribute to care planning.
• Assist with staff onboarding, training, and development.
• Maintain clinic inventory, equipment, and medication management processes.
• Support quality, compliance, and audit readiness initiatives.
• Participate in after-hours on-call and holiday rotation as needed.
