Job summary

Trinity Health PACE is seeking a RN to coordinate and supervise nursing services for participants in the PACE program, serving in the day center or on home care assignments. The role includes participation in IDT care planning and ensuring program operations meet regulatory standards.

The position requires rotating weekends and on-call duties, with strong leadership and communication skills, and a commitment to patient-centered care in a community-based setting.

Qualifications

• Must be a graduate of an accredited Nursing Degree program and hold an active RN license in the state.
• BSN preferred.
• Active CPR certification required.
• Minimum of three (3) years' health care experience with emphasis in geriatrics.
• Previous supervisory experience required.
• Valid driver’s license, insurance and use of personal vehicle due to home care assignments.

Responsibilities

• Coordinate and supervise nursing services in the PO, center and home care settings.
• Participate as a member of the interdisciplinary team (IDT) and participant care planning.
• Contribute to QAPI, budget planning and survey readiness; assist in plans of correction for department deficiencies.
