Qualifikationen

• BSN preferred.
• At least one year working with the frail elderly population.
• Five years of management experience preferred.
• Ability to lead and direct staff in the delivery of care.
• Must be medically cleared for communicable diseases and up-to-date immunizations.
• Excellent oral and written communication and strong interpersonal skills.
• Ability to document and present data using technology.

Aufgaben

• Collaborate with Physician and Nurse Practitioner to ensure optimal clinical flow.
• Oversee daily workflow and delegation of tasks to LPN, Medical Assistants, and Scheduling Clerks.
• Triaging and assessing participants and providing education resources.
• Supervise LPN and Medical Assistants to ensure competency.
• Ensure documentation of clinical activities in participants' medical records daily.
• Support quality initiatives and adherence to standards.
• Coordinate transitions of care and communications with RN Case Managers and interdisciplinary team.
• Support staff education and new hire competency training.
