Qualifications

• Baccalaureate Degree in Nursing from an accredited school or a Bachelor's degree in biological, health or social sciences.
• Licensed Registered Nurse in the State of Virginia or eligible.
• Minimum of three years of nursing experience in an acute care setting.
• Master’s Degree in Nursing or a healthcare related field preferred.

Responsibilities

• Works with attending and consulting physicians to facilitate effective transition through hospitalization.
• Collaborates with the multidisciplinary team to ensure patient needs are met and care delivery is coordinated across the continuum.
• Resolves psychosocial issues with social workers and develops complex discharge plans as needed.
• Acts as liaison between hospital and community agencies for exchange of clinical and referral information.
• Maintains compliance with CMS guidelines and QIO series; uses medical necessity criteria to assess care level and setting.
