Job summary

MidState Medical Center, part of Hartford HealthCare, seeks an RN Care Coordinator to join the interdisciplinary team at our Bristol, CT location. The role coordinates and monitors the plan of care to achieve optimal discharge planning outcomes, expediting value-added interventions and ensuring safe transitions for patients.

Current CT RN license with 3–5 years of clinical experience is required; BSN strongly preferred. CCM certification is a plus.

Qualifications

• Current CT RN License with 3–5 years clinical experience required.
• BSN strongly preferred.
• CCM certification strongly preferred.
• Excellent written and verbal communication skills and patient assessment skills required.

Responsibilities

• Coordinate and monitor the plan of care to achieve optimal discharge planning outcomes.
• Expedite value-added interventions to achieve desired outcomes.
• Serve as a change agent to improve care coordination and patient satisfaction.
• Collaborate with interdisciplinary team to support organizational and financial outcomes.
