Job summary

Central City Health, Inc. is seeking a Nurse Care Coordinator to serve as the primary clinical point of coordination for an assigned panel of adult members.

You will provide continuous oversight between scheduled visits, use nursing judgment, and proactively monitor changes in clinical and social needs while coordinating across primary care and behavioral health services. The role requires Michigan RN licensure, BLS, and strong experience in ambulatory or care coordination settings.

Qualifications

• Active, unrestricted Michigan RN licensure.
• BLS certification required.
• Strong clinical triage judgment, including medication reconciliation competency.
• Experience with EHR-based registry or panel management, and comfort with proactive patient outreach.
• Minimum two years' clinical nursing experience; ambulatory care, primary care, or care coordination experience strongly preferred.
• Boundary of triage vs provider sign-off reviewed by leadership before posting.

Responsibilities

• Coordinate care for an assigned panel of adult members with continuous oversight between visits.
• Monitor changes in clinical and social needs; perform medication reconciliation and clinical triage.
• Facilitate timely intervention and care coordination across primary care, behavioral health, substance use disorder, pharmacy, dental, and case management.
