Job summary

ALWP, through the Care Coordination Agency (CCA), seeks an RN Clinical Care Coordinator to manage clinical care for recipients. Responsibilities include assessments, ISP development, monitoring services, and coordinating with RCFE facilities and health teams.

The role requires CA RN licensure, CPR, and strong documentation skills, with travel to covered geographical areas as needed. HIPAA compliance and professional communication are essential.

Qualifications

• Must hold a current, unsuspended California RN license.
• CPR certification required.
• Proficient in computer use and electronic documentation.

Responsibilities

• Coordinate clinical services for ALWP recipients under CCA.
• Develop ISPs and monitor service delivery.
• Attend case conferences with RCFE staff and health teams.
• Maintain accurate charting and case records; document client contact.
