Job summary

Cherry Health is seeking an RN Care Manager to develop and implement individualized care plans for high‑risk patient populations in our Heart of the City clinic, Grand Rapids, MI. The role focuses on collaboration with the care team to optimize outcomes and reduce costs while ensuring compliance with policies.

The position is full-time and onsite, typically Monday–Friday, 8 am–5 pm, with responsibilities spanning assessment, care coordination, and patient education in a dynamic, patient-centered

Qualifications

• RN degree and Michigan RN licensure are required.
• Self-management support training completion is preferred.
• Three years of adult medicine or primary care experience within the last five years.

Responsibilities

• Identify high-risk patient populations based on referrals and risk stratification.
• Assess patients/families’ healthcare and psychosocial needs over time.
• Collaborate with providers to develop and monitor individualized care plans.
• Provide patient self-management support and empower families.
• Coordinate care across settings and continuity of care.
• Document timely follow-up, medication reconciliation, and care transitions.
