Job summary

Qualifications

• RN licensure in Wisconsin is required.
• Bachelor’s degree in Nursing is preferred.
• Four years of clinical health care experience as an RN required.

Responsibilities

• Screen candidates for case management and develop assessments and care plans.
• Review results and update care plans to reflect progress toward goals.
• Educate individuals and families about health conditions and resources.
• Coordinate and advocate for services across providers and settings.
• Facilitate communication among care team to control costs and improve quality.
• Support self-management with coaching techniques and motivational interviewing.
