Job summary

UW Health is seeking a Registered Nurse (RN) for the PCU – Progressive Care Unit in Madison, Wisconsin. The role combines on-site and remote work with occasional travel within the Madison area, focusing on home-based hospital care coordination and virtual patient management.

Requirements include a BSN preferred, 2+ years in acute care or home health, WI RN or compact license, and CPR/BLS. Prior EPIC and virtual care experience are advantageous, with competitive shift differentials enhancing

Qualifications

BSN preferred; RN license required in WI or compact state
2 years of acute care or home health nursing experience required
Current CPR/BLS certification required
Experience with IV therapy and venous-access devices preferred
Experience with care coordination and program development preferred

Responsibilities

Coordinate care for HBHC patients via virtual visits and phone communications
Conduct virtual medication administration visits and monitor patient conditions
Participate in interdisciplinary rounds and manage post-acute referrals and equipment coordination
Maintain thorough EMR documentation and ensure safe transitions of care
Hybrid role with onsite and remote work; limited travel within Madison area
