Job summary

UW Health in Madison, WI is seeking a Registered Nurse (RN) for Home-Based Primary Care. You will join an interdisciplinary team to coordinate care for adult and geriatric homebound patients, triage issues, and manage referrals via phone or video calls.

The role emphasizes care coordination, case management, and strong nursing skills. Eligible candidates will have 6 months RN experience; BSN preferred; licensure in Wisconsin is required.

Qualifications

• Graduate of school of nursing is required.
• Bachelor's Degree in Nursing or higher preferred.

Responsibilities

• Coordinate care for homebound patients as part of RN Care Coordination team.
• Triage emergent issues and manage referrals; communicate with patients/families via phone or video.
• Collaborate with MDs, APPs, Social Workers, Schedulers and other nurses to coordinate care.
